Revised Sept. 22, 1997

This package contains three text files (with .DOC extension). Two are tables of information that I couldn't get to fit into the NDB file with all my other information files. They need to be viewed in Memo with the margins (under Menu formaT) set to 1 and 80 and the screen ZOOMed to the smallest letter size.

The third file is a blank History and Physical form. I have had only limited success typing my H&P's on my palmtop. I can't type as I'm taking a history on this small a keyboard. I did use this form when I was with another physician (ie the Intern I was backing up as a resident) while he/she was taking the history. As they talked, I stood back and filled in the blanks. After we did the physical, I usually could leave a lot of the normal findings in and just type over the abnormal stuff. The labs are easy to do if you set the cursor to Overstrike rather than Insert. Finally, I have the cable from the HP Connectivity pack and a serial to parallel converter (from Educalc) to connect my palmtop to the nearest ward clerk's printer. I also had luck using a PCMCIA fax/modem card to fax the H&P to the nearest hospital fax machine.
